New patient registration guide to GP services

The practice covers a radius of approximately six miles from the health centre in all directions.

You will need a current medical card with part A completed and photographic proof of identity. It is therefore necessary to come in person to register. 

If you have lost or mislaid your card or are coming from abroad you can still register but in place of a medical card you will need to complete the necessary forms. 

If you are housebound the practice staff will make alternative arrangements.
